Commissioning Documents
- When filing court documents, you often must file affidavits as part of your court materials
- For Affidavits, you must swear or affirm that the information in your Affidavit is true before you sign it. This means you promise that the information in the document is correct. You do this by signing the Affidavit in front of someone who has the authority to supervise an oath or affirmation. This person also signs and dates the form and supporting exhibits.

- When an oath is being witnessed by commissioner of oaths, the deponent must answer the following question: “Do you solemnly affirm and declare that the contents of this affidavit as subscribed by you are true”
